You can open demat a/c lower than either CDSL or NSDL. Many bank pvt and public and many broking firms are allowed to conduct yourself as DPs (Depository Participants) you can approach them and ask them to open a Demat a/c for you. Some of the banks/brokers are Bank of India Stockholding, Standard Chartered Bank, CBoP, Share Khan, IL & FS, India Infoline, ICICI Bank etc.
You can stir to any of these and ask them to open a Demat A/c for you.

The documents reqd. would first and foremost be your identity proof (Driving Licence, Passport, PAN card, Voter Card etc) and for your residence proof: Ration card/ passport/ tel bill etc.
The most important document would be your PAN Card copy. And you'll also want to submit you bank a/c details for ECS or dividend payments etc.
